TO ALL WAKO MEMBERS

Please find herewith attached the official invitation for participation to the 1st part of the Senior World Championships (LC, LK, K-1) , to be held in Sarajevo, Bosnia and Herzegovina, from 19th to 27th October 2019 .

The invitation contains all necessary details you may need about programme, hotel and transfer booking, visa request ecc.
As second attachment you can also download the ENTRY FORMS in Word format to be used and duly filled in with number of your teams and for the hotel reservation, transfer booking and visa request .
Please read carefully the invitation and take note of the indicated deadlines for a good organization of the event .

Herewith attached you can find also important MEDICAL FORMS that according to the WAKO Medical Regulations each competitor will have to fill in and deliver at the moment of accreditation and official weigh-in:

1. WAKO Liability Waiver Form :
Must be signed by EVERY participant and official at WAKO Continental – World Championships;

2. WAKO Medical Questionnair e:
Must be filled out completely and signed by every competitor and official at the morning of weight control and medical check and handed in to the doctor, before they begin the medical check or register as a referee;

3. WAKO Medical Form : This form must be signed and stamped by a qualified doctor and brought with you when presenting yourself for Medical Check and weight control.

*** Special Note no group sign off is allowed, these documents must be signed and stamped for every individual attending the championships***

It is really important that you provide to distribute the above documentation to your medical teams and also your coaching teams, who in turn will have to inform all athletes of these documents and procedure.

It is responsibility of each National Federation to bring English language Medical Form, Insurance, and Liability Waiver .

Failure to comply with any of the above will result in the team being unable to participate in the Championship.

ATTENTION:
Aaccording to WAKO rules the eligibility for the participation to this WAKO World Championship is reserved only to WAKO National Federation/Association members.

For participation it is also MANDATORY to have duly provided with payment of the AFFILIATION FEE 2019 by bank transfer only before the event takes place.
