Dear WAKO members,
Please find here below the Medical Examination WAKO Questionnaire and WAKO Waiver for the athletes attending next WAKO World Championships Cadets & Juniors in Rimini-Italy, from 6 th to 14 th September 2014.
On both documents Parents or legal guardian Signature* is requested.
* Parents or legal guardian signature is mandatory for athletes under age of 18 th .
During accreditation (weigh in and medical control) each athlete must have both of these documents duly completed and signed by its parents or legal guardian.
It is compulsory also to have a valid medical certificate stating the athlete is fit to fight.
If parents or legal guardian’s signature is missing the President and/or Coach can sign in case they declare having legal authority from the parents of the athletes during the competition period .
